Why Health Home?
Like you, we at We Are One Family Health Services, know that some of the most valued people in our lives require special attention and dedicated care. Trying to provide that all by ourselves is certainly not easy; maybe even unsafe. It can certainly be rewarding and fulfilling, but it can also be overwhelming and all-consuming. If you or a loved one, needs assistance, we are here for you. We provide care to Adults, Children and Adolescents of all ages.
Who Is Eligible for Health Home?
Home Health are for people with Medicaid who:
Have two or more chronic conditions
Have one chronic condition and are at risk for a second
Have one serious persistent mental health condition
Chronic conditions include but are not limited to mental health, substance abuse, asthma, diabetes, heart disease, and being overweight.
Additional chronic conditions such as HIV/AIDS may be considered by the Centers of Medicare and Medicaid Services (CMS) for approval. And for those over 65 who qualify, our services are covered by Medicare and Medicaid.
Your Care Is Important to Us
For individuals with chronic illnesses, home may be the safest place to get care. In-home healthcare makes it possible to practice social distancing while also getting the essential, ongoing medical care that you and your family may need. And in-home care can help prevent unnecessary hospitalizations.

What is Health Home
Health Home is supportive care for individuals who have chronic conditions. Our Health Home program operates under a “whole-person” philosophy. We integrate and coordinate all primary, acute, behavioral health, and long term services and support to treat the whole person.

Services
Our team of health professionals provide Comprehensive care management, Care Coordination, Health promotion, Comprehensive transitional care/follow-up and referral to community & Social Supports.
Nurse Practitioners
Nurse practitioners can manage your health care as well as collaborate with other disciplines to provide effective and seamless care.
With a focus on illness prevention, management and maintaining quality of life, our nurse practitioners provide supportive services to manage symptoms, order and interpret diagnostic tests and therapies, prescribe and educate about proper medication use, and facilitate communication among all members of the healthcare team, including the client and family. Services are ongoing for as long as the client and medical provider feel they are necessary or helpful for the individual.
